Registered Nurse (Adult) – Care Home
About the home Well-run nursing home with a calm, person-centred culture supporting adults and older people with frailty, dementia, long-term conditions and palliative needs. Strong GP and pharmacy links, clear clinical governance, and good staffing ratios. The role You’ll be the clinical lead on your shift: assessing, planning and delivering safe, person-centred care, guiding care assistants, and keeping records inspection-ready. You’ll work closely with families and the wider MDT to maintain dignity, comfort and quality of life. What you’ll do Complete holistic assessments and create/maintain person-centred care plans Run safe medication rounds, manage PRNs, complete MARs and incident reports Lead on wound care, tissue viability, pressure-area prevention and falls reduction Deliver and monitor care for dementia, diabetes, COPD, heart failure and post-acute rehab Provide compassionate end-of-life care and symptom control; support families Coordinate with GPs, community nurses, therapists and pharmacists Lead and coach the care team; allocate tasks, run handovers and complete supervisions Maintain accurate clinical documentation and contribute to audits and CQC readiness Promote infection prevention and control, safeguarding, MCA/DoLS and whistleblowing What you’ll bring NMC-registered Adult Nurse (RGN) with a valid PIN. Recent experience in care-home or adult nursing (hospital or community also considered) Confident with clinical risk assessment, escalation and clear documentation Calm leadership, strong communication and a supportive approach to coaching HCAs Up-to-date mandatory training and commitment to CPD Nice to have: dementia care accreditation, verification of death, catheter/PEG care, syringe driver competency, falls prevention experience, tissue viability skills.
